Re-create Adobe Form in MS Word Form - oDesk

This is a simple way to make some quick money. I need help with re-creating a simple PDF form in MS Word, tweaking it, and finalizing as PDF form again.



Context: I created a scholarship application in 2014. It is an Adobe PDF Form that can be completed digitally. I now need to update this application to 2015, but would also like an editable version so that I can update it myself in future years.



Final output:

1) Editable form in MS Word, updated for 2015

2) Finalized version of form in PDF, updated for 2015



Requirements: proficiency with Adobe Forms and MS Word



To dos:

- Re-create PDF form in MS Word so that it is editable

- Change any references to "2014" to "2015"

- Finalize into an Adobe PDF form

- Send over both editable MS Word form and PDF form



Attachments:

- Final version of 2014 application in Adobe PDF form

- I also have an earlier version of MS Word form if you need it for reference (Note: not final version!), but can only attach one document right now.
